RULES OF REGISTRATION We call the attention of the participants to read and follow the rules that guide the registrations at IFHNOS 2010 World Tour. WAY OF PAYMENT Bank Slip* Fill out the registration form online, print the bank slip and make payment in any bank by maturity. After the pay deadline, the bank slip needs to be reprinted with a new deadline. Identified Bank Deposit* The registration can be made through IDENTIFIED bank deposit using the tax payer identification number (CPF) of the registree at Banco Itau - Branch: 0310 - C/C: 78008-5 - on behalf of SBCCP - SOCIEDADE BRASILEIRA DE CIRURGIA DE CABECA E PESCOCO - CNPJ (tax payer registration number): 49.332.307/0001-89. Credit Card** Fill out the registration form online, select the option to pay by credit card (VISA or MASTERCARD) and complete all requested information before finalizing and sending the registration form. *Only for Brazilians **For Brazilians and foreigns SCIENTIFIC PROGRAMMING The schedule of the Congress is subject to changes, both in relation to issues and regarding speakers. STATEMENTS Residents should send statement issued and signed by the institution to which they are linked with the end date of residence or course to the Executive Secretariat of the Event. Fax: (21) 2215.4476 E-mail: ifhnos@gapcongressos.com.br The participant who does not send the statement corresponding to his/her category until the day of the event must pay the difference to the value of inclusion in the category of Non member and non even member when he/she reaches the venue (R$ 550,00). DEALINE Pre-registrations can be made until Oct. 15th, 2010. The organizers are not liable for entries made outside the time limits specified in the Registration Form. PARTICIPANTS' MATERIAL There is a forecast of supply of material from the Congress to all participants but it is only guaranteed to those who do register by Oct. 15th, 2010. CANCELLATION AND REFUND OF REGISTRATION The cancellation and refund of enrollment (all categories) may only be made if requested until Oct. 15th, 2010 through e-mail ifhnos@gapcongressos.com.br along with the registree's bank account data. The reimbursement of registration will be made after the event being discounted 40% (forty percent) of the value on the administrative fee. Cancellation requests will not be accepted if made after Oct. 16th, 2010. TRANSFER OF REGISTRATION The transfer of registration may be requested until Oct. 15th, 2010 in writing via e-mail ifhnos@gapcongressos.com.br. If there is change of category the difference must be paid through bank deposit (if the value is greater) and proof faxed or e-mailed to the Executive Secretariat of the Event, or requested a refund (if the value is less) in the same transfer document. The repayment of this difference will be made after the event at a discount of 40% (forty percent) of the value on the administrative fee. COPY OF BADGE The organizers do not hold liable for the loss of the badge provided. The issue of the 2nd copy will be made only upon payment of 70% of the value on the venue of the event on the category of the participant. LOCAL SECRETARIAT The Local Secretariat will run from October 25th, 2010 at the Intercontinental Hotel for the removal of material for pre-registered congress members, receipt of new registrations and general information.
